Risk Management and Issue Resolution

Risk Management and Issue Resolution

A carve-out involves regulatory deadlines, data migrations, workforce transfers, and customer commitments—all running on a tight clock. A disciplined risk-and-issue process keeps surprises off the critical path and provides executives with clear options when problems emerge.

Risk-Management Framework

  • Establish a single risk register with unique IDs, probability (1–5), impact (1–5), and combined risk score.
  • Classify risks into operational, technical, financial, legal, and people categories.
  • Set risk thresholds: any score ≥ 12 requires a written mitigation plan and PMO oversight.
  • Review the top ten risks at every weekly PMO meeting; update status, owners, and target mitigation dates.
  • Track Key Risk Indicators (KRIs) such as open data defects, TSA exit slippage, and attrition rates.

Issue-Resolution Process

  • Log every unplanned event that has already occurred in a central issue log.
  • Triage within four hours: categorise as Severity 1 (critical), 2 (major), or 3 (minor).
  • Assign a single owner and expected resolution time; post updates twice daily for Severity 1.
  • Escalate unresolved Severity 1 issues to the Steering Committee within 24 hours.
  • Close issues only when root cause is addressed, not merely the symptom.

Governance and Reporting

  • Daily stand-up for Severity 1 issues until cleared.
  • Weekly PMO dashboard lists top risks, new issues, ageing issues, and residual exposure.
  • Monthly Steering Committee deck shows trend lines, mitigation effectiveness, and remaining risk-adjusted float on the schedule.

Risk-Mitigation Tactics

  • Parallel build or dual-run to de-risk single-point failures.
  • Pre-purchase spare hardware to reduce supply-chain lead times.
  • Lock change-freeze windows around cut-over to limit scope creep.
  • Secure provisional regulatory clearances in high-risk jurisdictions.

Deliverables

  • Master risk register with scoring, owner, mitigation status, and next review date.
  • Issue log with severity, ageing analysis, and resolution evidence.
  • Weekly RAG dashboard highlighting changes in risk exposure and issue backlog.
  • Post-mortem reports for any Severity 1 incident, including corrective and preventive actions.

Common Pitfalls

  • Mixing risks (future events) with issues (events already occurred) in one list.
  • Letting risks languish in “monitor” status without time-bound mitigation.
  • Closing issues without documenting root-cause fixes, leading to recurrence.
  • Failing to update risk scores after mitigation actions—gives executives a false sense of security.

A transparent, owner-driven risk and issue discipline turns uncertainty into manageable work, keeping the carve-out programme on track for an orderly Day 1 and timely TSA exits.
